Referral Packet

Connections Beyond Sight and Sound

Thank you for your referral to Connections. Please complete the enclosed Referral form and return it to me at the address below. Upon receipt of the completed form, a technical assistant will call you to schedule an initial visit.

The following is a summary of the Technical Assistance process:

Step 1 You complete the Referral form. Remember all signatures and sections. Send the completed Referral form to Donna Riccobono, Project Coordinator. Note: Referrals must be updated and re-signed for each student each school year.
Step 2 The Connections Project Coordinator assigns a Technical Assistant.
Step 3 The Technical Assistant calls to schedule an initial visit. This initial visit will include a full-day observation and a meeting with the educational team (including the parent(s), if possible). Note: For home-based programs, length of the initial visit(s) will be appropriate to the needs and preferences of the parent(s).
Step 4 At the initial meeting, a Memorandum of Understanding is completed and serves as the Technical Assistance Agreement. With the educational team, the Technical Assistant may write an Action Plan to identify the purpose, goals, tasks, timelines, people involved, evaluation strategies, etc. to meet your specific needs. Dates for subsequent monthly visits are scheduled, whenever possible.
Step 5 At subsequent monthly visits, further Needs Assessment will be conducted as needed. The Action Plan is implemented and monitored by the Technical Assistant and you.
Step 6 Evaluation of the Action Plan occurs. You and the Technical Assistant determine whether services should be continued or terminated based on progress towards meeting specified goals.
Step 7 At the completion of the Technical Assistance, you complete a final evaluations of the process and consultant’s work.
